Nasal Polyps Survey

Hi there I am on the bludge. I want you to help me help you by doing a nasal polyps survey.

The nasal polyps survey is only 6 questions and will only take between 1 -2 minutes to fill out.

My goal is to write an Ebook about my Nasal Polyps Journey (I have been nasal polyp free now for over 2 1/2 years) which I would like to share with you.

But before I get serious and put pen to paper I am reaching out to my followers to answer a few short questions and let me know their experience so far.

This way I can make sure my story will resonate and help as many people as possible.

The Survey Form is a Google Docs form and you don’t need to enter
your name or email to fill it out.
